Antidepressants and Phototherapy


Antidepressants are used commonly in medical and psychiatric practice. As a class, antidepressants have in common their ability to treat major depressive illness. Most antidepressants are also effective in the treatment of panic disorder and other anxiety disorders. Some antidepressants effectively treat obsessive-compulsive disorder (OCD) and a variety of other conditions (see indications below).

The most commonly prescribed antidepressants are listed in Table 12-1. Antidepressants are subdivided into groups based on structure or prominent functional activity: selective serotonin reuptake inhibitors (SSRls), tricyclic antidepressants (TCAs), monoamine oxidase inhibitors (MAOls), and other antidepressant compounds with a variety of mechanisms of action. Antidepressants are typically thought to act on either the serotonin or norepinephrine systems, or both. Choice of medications typically depends on diagnosis, history of response (in patient or relative), and the side-effect profile of the medication. Antidepressant effects are typically not seen until 2 to 4 weeks into treatment. Side effects must be carefully monitored, especially for TCAs and MAOls.

Indications

Table 12-2 lists the indications for antidepressants.

The main indication for antidepressant medications is major depressive disorder as defined by the Diagnostic and Statistical Manual of Mental Disorders, 4th edition (DSM-IV). Antidepressants are used in the treatment of all subtypes of depression, including depressed phase of bipolar disorder, psychotic depression (in combination with an antipsychotic medication), atypical depression, and seasonal depression. Antidepressants also are indicated for the prevention of recurrent depressive episodes.

Antidepressant medications may be effective in the treatment of patients with dysthymic disorder, especially when there are clear neurovegetative signs or a history of response to antidepressants.

Panic disorder with or without agoraphobia has been shown to respond to SSRls, MAOls, TCAs, and high-potency benzodiazepines (alprazolam and clonazepam).

OCD has been shown to respond to the serotonin-selective tricyclic clomipramine (Anafranil) and to SSRIs at high doses (e.g., fluoxetine at 60-80mg/ day). Obsessions tend to be more responsive to pharmacotherapy than compulsions. Symptoms of OCD respond more slowly than symptoms of major depression. Trials of 12 weeks or more are needed before a medication can be ruled a failure for an OCD patient.

The binging and purging behavior of bulimia has been shown to respond to SSRls, TCAs, and MAOls in several open and controlled trials. Because SSRIs have the most benign side-effect profile of these medications, they are often the first-line psychopharmacologic treatment.

Mechanisms of Action

Antidepressants are thought to exert their effects at particular subsets of neuronal synapses throughout the brain. Their major interaction is with the monoamine neurotransmitter systems (dopamine, norepinephrine, and serotonin). Dopamine, norepinephrine, and serotonin are released throughout the brain by neurons that originate in the ventral brainstem, locus ceruleus and the raphe nuclei, respectively. These neurotransmitters interact with numerous receptor subtypes in the brain that are associated with the regulation of global state functions including appetite, mood states, arousal, vigilance, attention, and sensory processing.

SSRls act by binding to presynaptic serotonin reuptake proteins, thereby inhibiting reuptake and increasing the levels of serotonin in the synaptic cleft.

TCAs act by blocking presynaptic reuptake of both serotonin and norepinephrine. MAOls act by inhibiting the presynaptic enzyme (monoamine oxidase) that catabolizes norepinephrine, dopamine, and serotonin, thereby increasing the levels of these neurotransmitters presynaptically.

These immediate mechanisms of action are not sufficient to explain the delayed antidepressant effects (typically 2 to 4 weeks). Other unknown mechanisms must play a role in the successful psychopharmacologic treatment of depression.

Migraines, Fibromyalgia, IBS, Neck Pain, TMJ, or Panic Attacks Out of Control? Make Lifestyle Chang

How can you take control of what is out of control? Begin with the basics. The basics mean making healthy lifestyle choices. Migraines, fibromyalgia, IBS, frequently neck pain, TMJ, and panic attacks are interrelated. The common denominator is migraine. I have chosen the phrase migraine syndrome to group the above symptoms together, because when you control migraines, then the migraine syndrome will improve. Successful migraine prevention depends greatly on understanding how lifestyle impacts the frequency and severity of migraine attacks.

People that are subject to the migraine syndrome have what I call the sensitive body. The body is more sensitive to light, sound, smell, food, and/or stress. It is important to remember that your body is sensitive. Avoid triggers. Triggers can be additive and total up to big trouble. The more trouble you have, the lower your threshold for developing symptoms. If your body is over-stimulated by triggers for a sufficient time period, then transformation of migraines occurs (migraine become more chronic) and more of the migraine syndrome is revealed. Transformation can ultimately lead to fibromyalgia. It is impossible to totally avoid triggers. Triggers are risk factors. Activities done to normalize the nervous system are protective factors. Lifestyle changes promoting protective factors and avoiding risk factors lead to good health.

Lifestyle changes markedly reduced the frequency of headache in studies that I have conducted. Let me prove my point. I have been the investigator in many medical research studies. In the late 1990's, I did two studies that I found extremely revealing about the importance of lifestyle changes. They were simple preference studies. The patients would try the study drug and determine if the study drug was preferable over whatever over-the-counter medicine they were taking for migraine. In one study Imitrex was the medication studied, and in the other Amerge was the study medicine. Imitrex and Amerge are triptans. Triptans are drugs that help stop a migraine attack. In one study Imitrex was taken for three headaches and in the other study Amerge was taken for four headaches. The study patients were required to have five or six headaches a month in order to be enrolled into the study. In other words, these study patients' lives were significantly impacted by having too many migraines. The patients had three months to complete the study. Assuming five to six headaches a month, then the study should only take a month. Nevertheless, they had three months. In both studies combined there were about seventy patients. My office treated these patients like regular migraine patients. We made the "mistake" of teaching the usual lifestyle changes—the changes I am about to teach you. Over half the study patients were unable to complete the study in three months! One patient did not have another headache in three months. Needless to say, we did not make that "mistake" again while conducting a study. We taught them lifestyle changes after the research projects were completed.

One of the basic needs is a consistent life. Everything needs to be done in a methodical way. Maintain adequate sleep—not too much and not too little. You need to go to bed at night and awaken in the morning at approximately the same time each and every day including weekends, holidays, and vacations. If you oversleep by more than one hour, you may feel hungover or tired, and you are definitely aggravating the migraine syndrome. Did you ever wonder why jet lag bothered you so much? Imagine awakening by 6:00 am weekdays and 8:30 or 9:00 am on weekends. That may explain weekend or Monday morning headaches or cricks in the neck. The fatigue is felt when you go on vacations and/or come home to reality. The variable sleep-patterns problems adversely affect high school and college students and night shift workers.

Eat regular meals. You need to eat breakfast! Commonly, I find migraineurs who just do not feel like eating early in the morning. Eat anyway. It does not have to be eggs and biscuits. A bowl of cereal or even a breakfast bar will work. Do not skip meals. If you do not eat, or if you eat junk food, you are setting yourself up for reactive hypoglycemia among a host of other problems. Consistent sleep and meals are very important. In other words, your life must be dull and boring—oops. What I meant to say is, the more regular your lifestyle is, the more protected you are from risk factors.

How To Know If You Are Burning Muscle Not Fats? Burn Body Fat Not Muscles


The danger of any weight loss program is that you may lose more muscles than fats. This is not healthy at all because when you lose muscles, your metabolism will slow down and your body will in turn retain more fats.

So how do you know if you are burning more muscles than fats? There is a simple and inexpensive way to find out.

Firstly, you must know that whenever your body burn fats for energy, a certain chemical call ketones are released. “When your body releases ketones, it is chemical evidence that you’re consuming your own stored fat.” said the famous MD Dr Atkins.

You want your body to burn the food you have eaten and even more so if you are obese, your body fat for energy and not your muscles.

One simple yet inexpensive way of knowing whether you are burning your body fat or muscles is to test for the production of ketones. If you are releasing ketones, you are burning fats instead of your muscles. If you are you are on a weight loss program and you are not producing ketones, then your program is not effective because you are burning other energy sources such as carbs or your own muscles. That means your weight loss program is virtually ineffective and can even be dangerous.

Ketones strips to test for ketones in your urine are readily available in most pharmacies. These strips are usually used by diabetics but are also a wonderful tool to check how effective your weight loss program is.

If you are losing weight and do not include exercises and a healthy diet in your weight loss program or you are actually on a severe caloric restriction diet, chances are very high that you may be burning your muscles for energy instead of your fats. What is Narcissism?


A pattern of traits and behaviours which signify infatuation and obsession with one's self to the exclusion of all others and the egotistic and ruthless pursuit of one's gratification, dominance and ambition.

Most narcissists (50-75%, according to the DSM IV-TR) are men.

The Narcissistic Personality Disorder (NPD) is one of a "family" of personality disorders (known as "Cluster B"). Other members of Cluster B are Borderline PD, Antisocial PD and Histrionic PD.

NPD is often diagnosed with other mental health disorders ("co-morbidity") - or with substance abuse and impulsive and reckless behaviors ("dual diagnosis").

NPD is new (1980) mental health category in the Diagnostic and Statistics Manual (DSM).

There is only scant research regarding narcissism. But what there is has not demonstrated any ethnic, social, cultural, economic, genetic, or professional predilection to NPD.

It is estimated that 0.7-1% of the general population suffer from NPD.

Pathological narcissism was first described in detail by Freud. Other major contributors are: Klein, Horney, Kohut, Kernberg, Millon, Roningstam, Gunderson, Hare.

The onset of narcissism is in infancy, childhood and early adolescence. It is commonly attributed to childhood abuse and trauma inflicted by parents, authority figures, or even peers.

There is a whole range of narcissistic reactions - from the mild, reactive and transient to the permanent personality disorder.

Narcissistic Supply is outside attention - usually positive (adulation, affirmation, fame, celebrity) - used by the narcissist to regulate his labile sense of self-worth.

Narcissists are either "cerebral" (derive their narcissistic supply from their intelligence or academic achievements) - or "somatic" (derive their narcissistic supply from their physique, exercise, physical or sexual prowess and romantic or physical "conquests").

Narcissists are either "classic" - see definition below - or they are "compensatory", or "inverted" - see definitions here: "The Inverted Narcissist".

The classic narcissist is self-confident, the compensatory narcissist covers up in his haughty behaviour for a deep-seated deficit in self-esteem, and the inverted type is a co-dependent who caters to the emotional needs of a classic narcissist.

NPD is treated in talk therapy (psychodynamic or cognitive-behavioural). The prognosis for an adult narcissist is poor, though his adaptation to life and to others can improve with treatment. Medication is applied to side-effects and behaviours (such as mood or affect disorders and obsession-compulsion) - usually with some success.

The American Psychiatric Association, based in Washington D.C., USA, publishes the Diagnostic and Statistical Manual of Mental Disorders, fourth edition, Text Revision (DSM-IV-TR), 2000.

The DSM specifies nine diagnostic criteria. For NPD to be diagnosed, five (or more) of these criteria must be met.

(In the text below, I have proposed modifications to the language of these criteria to incorporate current knowledge about this disorder. My modifications appear in bold italics.)

Feels entitled. Demands automatic and full compliance with his or her unreasonable expectations for special and favourable priority treatment;

Is "interpersonally exploitative", i.e., uses others to achieve his or her own ends;

Devoid of empathy. Is unable or unwilling to identify with, acknowledge, or accept the feelings, needs, preferences, priorities, and choices of others;

Constantly envious of others and seeks to hurt or destroy the objects of his or her frustration. Suffers from persecutory (paranoid) delusions as he or she believes that they feel the same about him or her and are likely to act similarly;

Behaves arrogantly and haughtily. Feels superior, omnipotent, omniscient, invincible, immune, "above the law", and omnipresent (magical thinking). Rages when frustrated, contradicted, or confronted by people he or she considers inferior to him or her and unworthy.

So what exactly is seperation anxiety?



Seperation Anxiety is actually an anxiety disorder that is defined through a state of intense panic induced from the dog's detachment from his owners.



So to put simply, when you leave the house, your boston terrier goes into a state of nervous anxiety which escalates exceedingly fast.



Boston terriers are social animals and need plenty of companionship to keep them happy and pleased. No boston terrier likes to be left alone for long amounts of time, but some handle it a lot worse than others.



The biggest cause of seperation anxiety for boston terriers is neglect. If you are gone a lot more than you are there in your boston terrier's life, separation anxiety is pretty much unavoidable. Your boston terrier requires your companionship, affection, and care to be pleased.



Symptoms of separation anxiety are pretty distinguishing. Your boston terrier will usually know when you are going to leave (he'll hear your keys jingling, see you putting on your coat, etc) and will get very anxious. He might follow you from room to room, whining, shaking, and crying. Some boston terriers even get aggressive trying to stop their owners from leaving.



Once you have left, the anxious behavior will quickly worsen and normally will peak within a half an hour. He might bark endlessly, scratch and dig at the windows and doors, chew inappropriate objects, and even urinate or defecate inside the house. In intense cases, he may even self-mutilate by licking or chewing his skin until it has become raw, or pulling out his fur.



Then when you return, he will be really excited, and will jump around you in a frenzy of delight for a lengthy amount of time (more than the usual 30 seconds of a pleased, well-balanced boston terrier.)



This prolonged greeting is a source of some misunderstanding. Without knowing that such a greeting actually shows a psychological disorder, a lot of owners actually encourage their boston to get more and more worked up upon their return (by stirring up their boston's excitement, encouraging him to leap around, and so on.)



Anytime you do this with your boston, you really need to stopshould really stop. I know that it can be tempting and very easy to do, and even seems harmless - but in actuality, you are just supporting his belief that your return is the greatest part of the day. So he is as happy as can be when you return, but when it's time for you to leave again, his happiness at your presence is under threat, and he gets even more worried when you leave.

Don't Get Burned By Summer Sun


(NC)-Warm summer days filled with bright sunshine aren't always good for you. Over exposure to sunlight can cause serious sunburn, pain and the long-term risk of skin cancer. St. John Ambulance, Canada's leader in safety-oriented™ first aid training and products, points out that you can still enjoy sun if you reduce risk by taking precautions.

People with fair skin are most susceptible to sunburns. However, no matter what the skin type, the harmful effects of the sun, such as sunburn, can be sustained by anyone.

Prevention is your best defense. Use a sunscreen strong enough to block out harmful rays. Remember to apply it liberally many times over the course of a day in the sun as it will soak into skin, evaporate and be washed off in water.

Don't stay outdoors for extended periods without covering exposed skin. Never leave tender skin exposed for more than a few minutes at a time. Wear a light, sun repellent cover over bathing suits or other light clothing. Always keep your head and eyes protected.

First Aid for sunburn

Sunburns can be very serious. Ranging from mild discomfort to severe burning over a large portion of the body, extreme sunburn can be further complicated by heatstroke. For minor sunburn, St. John Ambulance suggests the following first aid:
  1. Check the casualty thoroughly to determine the extent and severity of the burn.
  2. Get out of the sun immediately.
  3. Cover the burn with a wet towel or gently sponge the area with cool water to relieve pain.
  4. Pat the skin dry and apply medicated sunburn ointment or lotion. Apply according to directions on the package and watch for warning signs of an allergic reaction.
  5. Protect burnt areas from further exposure to the sun.
  6. Don't break blisters - doing so may promote infection. If large areas of the skin begin to blister, seek medical help.
  7. If the casualty begins to vomit or develops a fever, give first aid for heatstroke and get medical help.


First Aid for heatstroke

Heatstroke is a life-threatening condition in which the body's temperature rises far above normal. The body's temperature control mechanism fails, sweating may stop and the body temperature rises rapidly. When you notice rapid pulse, noisy breathing, convulsions or vomiting and hot, flushed skin, give first aid for heatstroke.
  1. Check the casualty for symptoms of heatstroke. Lowering the body temperature is the most urgent first aid for heatstroke.
  2. Move the casualty to a cool, shaded place or indoors if possible. Call for medical help.
  3. Cool the casualty by removing clothing, covering with a wet sheet, immersing in cool water or sponging with cool water, especially in the armpits, neck and groin areas.
  4. When the body feels cool to the touch, cover the casualty with a dry sheet. If temperature begins to rise again, repeat step three.
  5. Continue to monitor the casualty until medical help is available.

Did You Overeat On Turkey Day??


Lose weight before Christmas!

It’s that time of year again when most of us (myself included) forget about the gym, our diets, and fitness routines and start to let ourselves go. Yes, us men even though many won’t admit it publicly care about what we look like and yes even a few of us go on fad diets. The number of men who are getting plastic surgery is at an all time high. The largest cause of epilepsy, however, is hereditary or genetic.

Any person can endure an epileptic seizure under certian circumstances. The seizures can be provoked by drug overdoses, or in some cases even acute illness. The condition epilepsy, however, does not apply to these people. Epilepsy is defined by recurring, unprovoked seizures, although there is controversy over symptoms required to be diagnosed with the disease.

Only 1 percent of the worlds population suffers from epilepsy according to its criteria, however, it is estimated that there is a slightly higher percentage of actual conditions. Most of this percentage is found in persons under the age of 19, or over the age of 65. The diagnosis criteria for epileptics is widely controversial in the field of medicine. Be Thankful for What You Have Got


Did you know that the 911 equivalent in the UK (999) now has call waiting? I hope that you never find out for yourself.

Have you ever had to resuscitate someone? I sincerely hope that you haven’t. Have any of your family spent time in hospital? If they have, I hope it wasn’t for long.

Well, I have experienced all three of the above the following number of times in the last three years:

Held in a queue after dialling 999 - 4 times
Given resuscitation to my son - 6 times
Time spent at hospital bedside - at least 18 months

Oh, I almost forgot. We have also been told on no less than three occasions that it would be unlikely he would survive the next 24 hours.

Am I bitter? No. In a way, I’m grateful for the experience. I wouldn’t be doing what I am doing now if it wasn’t for my son. All of my priorities have been turned around because of him.

I could be one of those people who wallow in self-pity at how unlucky they are, or how unfair life is. I have chosen not to. I have chosen to take something positive out of the fact that my 3-year-old son is severely disabled, and will be for the rest of his life. I have quit my job to spend time looking after my family. I now spend more time with my wife, my son, and I can take my 6-year-old daughter to school every day. I have decided that life is there for living, and that money (although nice to have) shouldn’t be the prime motivator for working.

I can also now appreciate how lucky I really am.

There are many families who have a ‘special’ child like ours, who is not as fortunate as my son. We are lucky that we have a healthy daughter. I am lucky that I have a wonderful wife. Being in this situation makes you appreciate the good things, instead of focusing on the bad things.

I envy families that can go on holiday together, who can play in the park together, or can go out for a meal together. The saddest thing is not what I haven’t got, it is those who have got what I want not even realising the lucky hand they have been dealt in this game of life. Be thankful what you are able to do or be, and don’t dwell on those things that you wish for. If you could realise how lucky you are, then your life will improve dramatically, just like mine. Be grateful that you have a roof over your head, electricity, television, and running water. There are a lot of people in this world who have none of these and end up becoming more successful than you or I.

If you can show gratitude for what you have, and use your energy towards attaining what you don’t have, then you are on the first rung on the ladder to success.

Can a Neurotransmitter Imbalance Be Causing Your Mood Problems??


Neurotransmitters are powerful chemicals that regulate numerous physical and emotional processes such as cognitive and mental performance, emotional states and pain response. Virtually all functions in life are controlled by neurotransmitters.

Benzodiazepine Withdrawal Treatment Guidelines

I spoke to my doctor in the beginning of the week regarding my Klonopin withdrawal and Prozac withdrawal. Because of all the symptoms and bizzare feelings I have been having, I asked if I should be in a rehab at this point. He said no, it would not be appropriate, and that rehab is recommended for acute withdrawal.



I spoke to friends who have been through rehab for prescription medication and also did some research of my own. There is a wonderful video about benzodiazapene withdrawal treatment guidelines presented by Dr. C. Heather Ashton, the leading expert on benzodiazepines and benzodiazepine withdrawal.



Here are the main points:



•There is no single protocol for benzodiazepine withdrawal. Drug dosages should be tapered gradually and the withdrawal should be individually tailored for each patient.



•There is no reason for abrupt withdrawal, in fact rapid detox (such as in a rehab) is not recommended. Especially from high doses, can result in seizures, psychosis, and severe panic attacks.



•Can take months or up to a year or more.



•The patient and doctor initially agree on a withdraw regimen or schedule, but for the best results, the patient should be in control of the rate of withdrawal. (note

to self: slow the hell down)



If problems and setbacks happen, the patient may do one of 2 things:



1. Stabilize the medication dosage for a few weeks or

2. Decrease the rate of withdrawal (revise the withdrawal regimen)



The goal with benzodiazepeine withdrawal is to always go forward and avoid a backslide of increasing the dosage of the drug. Living without medications is a learning process that may take months. Once done you can stop panic attacks without any medications.

1. Relaxation Techniques - Relaxation is the antithesis of anxiety. A patient suffering from anxiety has difficulty putting his mind at rest and keeps worrying about one thing or the other. Common sense, therefore, dictates that relaxing your mind can be a very effective antidote to anxiety.

You should try meditating, preferably in a quiet environment. Light a few candles, put on some soft, relaxing music (nature sounds are very effective in this). Fold your legs under your hips (the meditative stance) and breathe in deeply with your eyes closed and try and focus on pleasant thoughts and memories. If possible, join a Yoga class - it'll help you tremendously in relaxing your mind.


2. Breathing Techniques - One of the most common physical symptom of an anxiety/panic attack is difficult, rapid breathing, called hyperventilation. Hyperventilation increases the oxygen level in the body and reduces the carbon dioxide levels, which can lead to a feeling of light headedness or even cause you to faint.

A person suffering from an anxiety condition should try and breathe through his diaphragm (this is the bony part of your chest that moves up and down as you breathe, right above the abdomen). Also, try taking in slow, deep breaths. This can help you tremendously in controlling your anxiety.


3. Adjustments in Diet - Certain diet elements, such as nicotine, alcohol, caffeine or other stimulants can cause an anxiety attack since they trigger adrenaline release in the body. Adrenaline is one of the main stress chemicals. Oily, heavy foods should also be avoided. Try eating a light, protein and carbohydrate rich diet with plenty of green vegetables and fruits.

4. Exercise - Exercise can improve your mood exponentially. Exercise helps to burn stress chemicals and eases the mind. A jog through a park or a long bike ride can be a great way to improve your mood and keep anxiety at bay. Make daily exercise a part of your routine. Not only will it help control anxiety, it will also help check your weight, giving you a better self image and more confidence.

Does The Place You Live Affects Your Wellness?


During the course of your growing up years, you lived wherever your parents chose to live. You didn’t give any thought to the health implications of the location your parents chose, or if they had chosen a place that was conducive to your overall wellness. Chances are your parents didn’t give it much thought either. Not until recently, has there ever been given any thought to the fact that where you live affects your health and wellness. But it does, and it’s a piece of information that is sure to influence many generations to come.

So how is this information compiled, and what can we learn from it? The information is compiled based on statistical information from areas such as smog levels, pollution levels, water quality, government based fitness incentives, and recreational and fitness facilities available. What do we learn from all this published information? That where we live really does affect our health and well-being, and sometimes, there’s very little we can do about changing that fact. Unless, of course, you want to move.

Conquering Fear: Step Away From The Panic Button

Where all else fails to stop us from achieving what we want from life, fear steps in. We experience fear on both conscious and unconscious lelimiting emotions we possess. In some cases fear is justified, and even healthy. For example, a person contemplating crossing a busy street will harbor a healthy fear of being struck by two tons of rapidly moving steel commonly known as a motor vehicle (at least, if he or she is a reasonably sane person who understands the basic laws of physics: moving car + walking person = splat). This fear breeds caution, which causes the person to look both ways for oncoming traffic and wait for an appropriate time to venture across the road.

However, unjustified fear- which can be just as crippling and realistic as justified fear- is more often the case when fear is a factor. Not many people risk their lives on a regular basis.

Humiliation, rejection, and failure top the list* of limiting fears that can be overcome with practice and determination.

* Actually, spiders top the list of fears for most people. Arachnophobia- fear of spiders- is the most common type of fear in the world. However, fear of spiders is completely justifiable, as spiders are creepy eight-legged insects with fangs, alien eyes, and a tendency to drop on you out of nowhere.

One of the easiest and most successful methods of dealing with fear is exposure therapy, which is actually facing your fears one small step at a time. If you don't feel you can handle exposure therapy alone, enlist a friend to participate- especially if you can find a friend who doesn't fear the same things you do. With exposure therapy, the objective is to experience fear to a small degree several times, so that each time it becomes easier to conquer. (Please note that exposure therapy does not apply to every situation. For example, if you are afraid of flying, it is not recommended that you leap from successively higher perches and attempt to become airborne.)

Here are some ways you can implement exposure therapy for the Big Three fears:

Humiliation

" Wear your slippers to the grocery store. If you're feeling ultra-brave, scuff your feet across the floor to call attention to your slippers. If you're feeling ultra-timid, go to a grocery store far enough from your house that the shoppers will probably never see you again.

" Sing at a karaoke bar. While you're sober.

" Choose one completely inappropriate article of clothing (a Dr. Seuss hat, a big pair of fuzzy mittens in the middle of summer, a headband with bumblebee antenna) and wear it in public as long as you can. This is not only good exposure therapy- it's fun!

" Join a local Toastmasters club or offer to give a public presentation on an area relating to your expertise at a library or school. Public speaking is an excellent channel for exorcising humiliation, especially if you do it on a regular basis (that's speak in public, not humiliate yourself).

Rejection

" Call up a deejay at a local country radio station and request a song by Metallica or Ozzy Osbourne. Be aware that you will be rejected; you might be laughed at and rejected, and there is a possibility you may be laughed at and rejected on the air.

" If you're single, use an online location service like Classmates.com or PeopleFinder.com to find an old school classmate you used to have a crush on. Contact them and ask for a date (or just initiate a conversation). If you're married, contact an old school classmate and invite them to lunch. At worst they'll say no; at best, you will have rediscovered a friend.

" Write a poem or a short story and try to submit it to a newspaper or magazine, or enter a writing contest. If you aren't rejected, become a writer immediately.

Failure

" Try to nail Jell-o to a tree.

" Buy a new video game and attempt to win it in one sitting. If you play video games on a regular basis, buy a video game that's different from the ones you usually play (for example, if you enjoy fighting video games, try a quest-driven format. Or video chess.).

" Start a new hobby that requires creating an end product, such as knitting, model kit building, or cake decorating. Please note that if you are working on your dietary habits, it is not advisable to embark on cake-decorating exposure therapy to combat fear of failure. You will feel obliged to consume your failed attempts. Instead, try vegetable sculpture or fruit bowl arrangement.

" Challenge Jeff Gordon to a stock car race. This will also help overcome your fears of rejection and humiliation, as at least one of them is bound to happen.

Conquering your fears is like climbing a mountain - do it one step at a time.

You can determine your own form of exposure therapy by coming up with ways to face your personal fears one small step at a time. If you can't think of anything, ask a friend to help. Most people are more than willing to try something new, especially if they get to watch you do something entertaining.

NOTE: These exercises are not intended as a substitute for professional psychiatric care. If your fears are extraordinarily strong and interfere with normal functions or daily activities, you should seek the advice of a certified psychiatrist. Self-induced exposure therapy can be effective in reducing or alleviating normal fear, but should not be used in cases of mentally crippling or trauma-induced fear.
